Health & Wellbeing Hybrid Hubs

Our Health & Wellbeing Hybrid Hubs, in partnership with SCORE PCN, provide essential health support directly into Scarborough’s communities.
Delivered in trusted, non-clinical settings, the Hubs offer blood pressure checks, health conversations, smoking cessation support, diabetes guidance and wider wellbeing advice - reducing barriers to access and helping people take control of their health earlier.
Over the past year, we delivered 112 pop-up sessions across 37 community locations, supporting over 800 residents.
Many of the people we see would not otherwise access traditional healthcare due to anxiety, transport costs, long waits or previous negative experiences.

Reliable access

By meeting people where they are, we provide early intervention that prevents crisis and reduces pressure on GP surgeries and urgent care.
To ensure regular, reliable access to community health support, we now run two weekly Health Hub sessions in Barrowcliff and Eastfield as well as our regular hubs in various locations.
These sessions offer a consistent point of contact for residents, enabling ongoing monitoring, timely advice and supported referrals into NHS and community services.
Our hybrid model blends practical health checks with supportive, person-centred conversations - helping residents understand their health, make informed choices and feel more confident engaging with services.
Working in partnership with NHS teams, local authority services and community organisations, the Hubs create a joined-up approach that strengthens prevention, improves wellbeing and builds healthier neighbourhoods.
